# Patient page navigation

Click a patient's name from anywhere in the platform to open their Patient Page — a comprehensive view for personalized care management.

## Sections

### Overview

Displays unresolved Action Items and the patient's Care Program status, including diagnosis-specific information. Use this as your starting point to understand what needs attention.

### Patient journey

A comprehensive, longitudinal view of all text message interactions and scheduled messages throughout the patient's program. Use this to understand the full arc of the patient's engagement.

### Surveys

Shows surveys that have been sent to the patient along with:

* Completion status
* Individual responses
* Resulting Action Items

### Patient info and settings

Contains the patient's demographics and communication preferences. Fields can be edited in real time directly from this section.
