A summary of unresolved Action Items and the patient’s status in the Care Program, including diagnosis-specific details.
Patient Journey
A comprehensive, longitudinal view of all text message interactions between the platform and the patient. You can see all messages scheduled for a patient throughout the program.
Surveys
View all surveys that the platform has sent to the patient, their completion status, individual responses, and any resulting Action Items.
Patient Info & Settings
Shows the patient’s demographics and communication preferences. If you need to edit the patient’s information, click the edit button from any tab to make real-time updates.
